North American offers the following online service forms for your convenience. When you've completed the selected form(s), please print, sign, and date the form(s) and mail to the address indicated at the bottom of the form(s).

Beneficiary Change
- Beneficiary Change (L-2402) - To change to whom the death benefit is payable
- Certification of Trust (L-3172A) - Additional requirement when naming a trust as beneficiary
Multi Purpose Change
- Multipurpose Service Request (L-2777) - Multipurpose service request form (Address Changes, Name Changes, Automatic Premium Loan, Policy Certificate/Duplicate Policy)
- Address Change (L-2777) - To change the owner's address of record
- Policy Loan/Partial Withdrawal/Policy Surrender (O-2811)
- Children Insurance Rider - To add a child(ren) to existing rider on policy
- Policy Change Request (Non-Underwritten) (O-2809) - For conversion, removing/reducing benefits.
Ownership Change
- Ownership Change (L-2402) - To change the owner of the policy
- Certification of Trust (L-3172A) - Additional requirement when naming a trust as the owner
- Collateral Assignment Change (L-776) – To place a policy assignee
Premium Payment Change
- Electronic Funds Transfer Authorization (L-1683) - To set up or change bank information on a policy
- Credit Card Authorization Form (L-2856) - To set up or change credit card information on policies. Please note this option is only available for term life insurance policies
- Change Mode of Premium Payment - To change frequency of payments (L-2777)
